Statement of Understanding and Compliance Form

NOTE: This task used to be the “Owner Statements of Designated Underground Storage Tank (UST) Operator
and Understanding of and Compliance with UST Requirements.” As of October 2018, the task has been divided into the “Statement of Understanding and Compliance Form” and the “Designated Underground Storage Tank Operator Identification Form” Complyrs tasks reflect the change. However, CERS will not incorporate the change until April 1, 2019. If you need to submit an update to any of these forms, we suggest you upload both of the new forms to the “Owner Statements of Designated Underground Storage Tank (UST) Operator
and Understanding of and Compliance with UST Requirements” task on CERS and update after April first by re-uploading to the new separated task.

The Statement of Understanding and Compliance Form is a task in the Underground Storage Tank portion of the California Environmental Reporting System compliance program.

Overview

Underground storage tank owners and operators must complete the The Underground Storage Tank Statement of Understanding and Compliance Form. The specific wording of the form is as follows:
I hereby certify that I understand the underground storage tank requirements of Health and Safety Code, division 20,
chapter 6.7, California Code of Regulations, title 23, division 3, chapter 16, and any applicable local underground storage
tank ordinances and that the facility identified above is in compliance with all applicable underground storage tank
requirements.
The specific requirements of The Underground Storage Tank Statement of Understanding and Compliance Form are also written on the form:
Every underground storage tank (UST) facility must submit a one-time statement indicating that the owner or operator
understands and is in compliance with all applicable UST requirements. A copy of this completed form must be submitted
via either the California Environmental Reporting System (CERS) or an equivalent local Unified Program Agency electronic
reporting portal within 30 days of: 1) an installation of a UST; or 2) a change in owner or operator of the UST, as applicable.

The top portion of the form identifies:

  • Reason for submitting the form (New Installation, Change of Owner, Change of Operator.)
  • Facility Name
  • Facility ID# (This can be the local agency permit number or your CERS ID#. Consult with your local Unified Program Agency to see what they expect.)
  • Facility Address
  • Facility Phone
